6th Grade Student Requirements
Physical exams are required for all 6th grade students.
7th Grade Student Requirements
Tdap and MCV immunizations must be completed prior to the beginning of school or risk exclusion.
Dental exam are required for all 7th grade students.
Physical and Dental Forms
Physical and dental forms can be found on the left under the Forms tab.
It is always best to have the exam completed by your family physician and dentist.
State Mandated Health Screening Program
Height, weight, BMI* and vision screenings every year
Hearing screening grade 7
Scoliosis screening grade 6 (with physical exam) & grade 7 (See Scoliosis pamphlet linked below)
